What Are Personality Disorders?

The Diagnostic and Statistical Manual of Mental Disorders (DSM-5) categorizes personality disorders into three clusters: 

Cluster A: Odd or Eccentric Disorders 

These disorders are characterized by unusual thinking or behavior that may seem socially detached or paranoid. 

Includes: 

  • Paranoid personality disorder – distrust and suspicion of others 
  • Schizoid personality disorder – emotional detachment and limited social interaction 
  • Schizotypal personality disorder – eccentric behavior, magical thinking and social anxiety 

How PHP/IOP Can Help: 

  • Structured therapy that builds social skills and trust 
  • Support for managing social isolation and anxiety 
  • Psychoeducation to help individuals understand their thought patterns 
  • Cognitive behavioral therapy (CBT) to challenge distorted beliefs 

Cluster B: Dramatic, Emotional, or Erratic Disorders 

These disorders involve intense emotions, impulsivity and unstable relationships. 

Includes: 

  • Borderline personality disorder (BPD) – emotional instability, fear of abandonment, self-harm 
  • Antisocial personality disorder (ASPD) – disregard for rules and the rights of others 
  • Histrionic personality disorder – attention-seeking behavior and extreme emotional expression 
  • Narcissistic personality disorder (NPD) – inflated self-image, lack of empathy 

How PHP/IOP Can Help: 

  • Dialectical behavior therapy (DBT) for emotion regulation and impulse control (especially effective for BPD) 
  • Group therapy to practice safe and respectful interpersonal communication 
  • Family therapy to repair damaged relationships and set healthy boundaries 
  • Crisis prevention planning and self-harm support where needed 

Cluster C: Anxious or Fearful Disorders 

These disorders are rooted in deep anxiety, fear of rejection and avoidance. 

Includes: 

  • Avoidant personality disorder – social inhibition and feelings of inadequacy 
  • Dependent personality disorder – excessive reliance on others for reassurance and decision-making 
  • Obsessive-compulsive personality disorder (OCPD) – preoccupation with control, order and perfectionism (distinct from OCD) 

How PHP/IOP Can Help: 

  • Exposure and response prevention for social fears or rigidity 
  • CBT to challenge negative self-perceptions 
  • Skills training to increase independence and decision-making 
  • Supportive group therapy to build confidence and reduce isolation 

Why IOP and PHP Work for Personality Disorders 

Unlike weekly therapy sessions, intensive outpatient programs (IOP) and partial hospitalization programs (PHP) provide frequent, structured and comprehensive care without requiring inpatient hospitalization. These levels of care are especially helpful for individuals with personality disorders because they: 

  • Offer daily support and routine 
  • Teach real-life coping skills 
  • Reduce crisis moments through ongoing monitoring 
  • Provide a safe environment to explore deep-rooted emotional issues 
  • Include group therapy for interpersonal skill-building
